PURPOSE

Give the crew one method for flying a table-form from floor to floor: designated pick points, no pulling the table out, confirmation-loop radios, holds. Formwork carpentry stays with the host.

SCOPE

Operator, rigger and signalperson on a flytable / flyform cycle. Not a how-to for dropping jacks, rolling dollies or landing props. Those belong to the manufacturer cycle and the site-specific engineered drawing. Corner and nontypical tables: GOSPEL-SWP-029 on top of this procedure.

This document is the method — the steps. Hazards, consequence and residual risk for the same work are in the JHA library. A procedure is not a hazard analysis. A hazard analysis is not a procedure.

PROCEDURE

  1. 01

    GATE

    Engineered drawing for this table ID and this floor. Manufacturer cycle at the lift. Weight and COG known (14.36). Pick points and sling lengths as the drawing. Wind — the lower of crane, manufacturer, this SJP. GOSPEL-SJP-001 / FRM-052 filled. Critical lift plan. Radios tested, confirmation loop, both floors. Pre-lift meeting immediately before this cycle. Corner or nontypical: GOSPEL-SWP-029 and that plan. If any of that is missing, the hook does not take the table.

  2. 02

    DROP

    Formwork drops the table per the manufacturer and the drawing — jacks, lowering devices, reshores. The hook is not a substitute for lowering jacks. Confirm the table is free of the slab, pins and spindles as the OEM, nothing snagged. Crane on standby. Do not take load to strip.

  3. 03

    ROLL AND ATTACH

    Dollies, glides, casters as the manufacturer. Curb stops and kicker blocks at the edge. Brake lines if the drawing requires them. Fall protection at the unguarded edge (Part 11). Exclusion below. The crane does not pull the table out of the building. Attach to every designated pick point — pins, cotters or Safety Pin-Bolts as the OEM — before the table leaves the slab. People off the table. Loose material off the table.

    FLY

    Test lift. Confirm the hang as the drawing — some systems use a longer rear pair, only if that drawing says so. Wind. Path. Tag lines. Confirmation-loop radio, both floors. Hoist clear, then up. A table is a sail. Stop on any change.

  5. 05

    LAND

    Guide onto the landing floor. Set on jacks or landing dollies as the manufacturer. Do not disconnect until the table is stable, tiebacks and curbs as the drawing, and the formwork supervisor says it is landed. Then release.

  6. 06

    AFTER

    Inspect table and rigging. Record the cycle. The next cycle starts from the drawing, not from memory.

RESPONSIBILITIES

This program

The published method for the work. Stop-work is named in it. It does not own the site.

Supervisor

Confirms the plan, the crew, the zone and the stop criteria. Owns whether the hook moves.

Crane operator

Runs only the machine they are competent on, to the chart, after inspection. Stops on an unclear signal or a broken plan.

Rigger

Selects, inspects and applies the hitch. Does not guess weight, centre of gravity or angle.

Signalperson

One set of hands on the crane. Holds the zone. STOP means stop.

Worker

Works to this program and to site rules — whichever is stricter. Uses the three rights. Reports damaged gear.

Formwork supervisor (host)

Drops, rolls and lands the table to the manufacturer cycle and the engineered drawing. The crane crew does not lower jacks or invent a cycling sequence.

Prime contractor

Coordinates floors, drawings, inspections and exclusion. Not a spectator. WorkSafeBC names this duty on flytable work.

REQUIRED COMPETENCY

  • BC Crane Safety certificate for this crane class
  • Critical-lift brief — everyone can repeat the abort
  • Knows the pick points on this drawing, not last floor’s memory

  • Fall protection where the work exposes a fall

PROHIBITED PRACTICES

  • Using the crane to strip or drag the table out of the slab
  • People or loose gear on the table while it moves or flies
  • Improvised pick points, missing pins, or a two-point fly on a four-point table
  • Flying a corner or nontypical table on the typical SJP
  • Changing the sequence without the engineer
  • Skipping a curb, tieback or brake line the drawing names
  • Last floor’s SJP for this table

EMERGENCY RESPONSE

Table hung up, rolling toward the edge, dropped object, person in the zone, wind pickup, lost radio — stop. Land if it can be done without a second incident. Do not pull. First aid. Notify the site and the employer. Hold the scene.
